Your First Visit!
Your first visit to Broadway Chiropractic will give you the opportunity to see if Chiropractic care can help you. In order to help you reach your health goals, your initial appointment will consist of a consultation, an examination, and a report of findings with a recommended treatment plan.
Initial Paperwork
Before you meet with Dr. Uthlaut, we will need you to
complete health history and information forms. To save your
time, these forms can be downloaded from this website, filled
out, and brought in with you. Or, you can fill them out in our
office. Our friendly receptionist will be happy to help with any
questions or concerns during this process.

Consultation
You will meet personally with Dr. Uthlaut regarding your present condition and any other concerns about your health. Knowledge of your past and present health conditions will enable us to create a treatment solution that will put you back on the path to recovery.
Examination
After your consultation, Dr. Uthlaut will perform a complete
chiropractic examination, including neurological, orthopedic,
physical and postural examinations, to determine if yours is a
condition that can be helped with chiropractic care. If
necessary, Dr. Uthlaut may recommend x-rays to get a
through evaluation of your condition.

Treatment
With your consent, Dr. Uthlaut will usually complete your
initial appointment with your first treatment, which may
include spinal adjustments, physical therapy, and
recommendations for things you can do at home to
augment your care.
We encourage you to voice any questions or concerns you may have with your
suggested treatment schedule so that we can ensure you have a pleasant
chiropractic experience.

Report of Findings
Once the examination is complete, Dr. Uthlaut will go over
his findings and make recommendations to reach your
health goals. It is very important to us that you understand
what we are doing, as we want to foster an open and
interactive doctor/patient relationship, in your health care.
